(backorders are not included in these costs) Wholesale Account Requirements: Download Application (.pdf formated) A copy of your business license. A copy of your sales tax license. A copy of your business check. A copy of a business advertisement. (i.e. Stained glass yellow page listing, online yellow page listing, craft show vendor listing, class instructor listing, newspaper advertisement, sales flyer, or your website address) Initial Order of a minimum of $500. Each application will be individually reviewed. Please allow us at least 2 business days to verify your application and information.
